Thank you for your interest in CostumeRentals. In order to process your inquiry request, we ask that you complete and submit the following forms. We will confirm receipt of your materials and give you a preliminary response within 1-2 business days. We will do our best to work with your production schedule, but depending upon the size & complexity of your request and the volume of orders we are processing, it may be 1-2 weeks before we can let you know exactly what we have available to fit your needs.
- Rental Client Information Sheet — This tells us about who will be renting so we can determine the appropriate rental rate. Once we have this form on file you will only need to send in the Project & Contact Information Sheet for future rentals.
- Project & Contact Information Sheet — This tells us who is working on the project, what, and when it is.
- Shipping Agreement — You may select your shipping speed and provide your shipping account number, or shipping charges can be added to your invoice.
- Measurement / Costume request sheet — Please provide your actors' measurements and costume request specifics. (You may send this in your own format via fax, or e-mail.) You may also send renderings and additional research materials for us to review. This will help us select the costumes which will work best with your particular production.
